What is ESP2?

Multi-centre trials provide key evidence underpinning healthcare practice. However, they invariably take a lot of hard work and expense to complete, with some of this work and expense being devoted to sites that under-recruit. Developing practical methods to identify sites that will recruit to target would be helpful.

ESP2 aims to evaluate a guided site performance prediction tool for trial managers. The tool was developed during the initial ESP study using practical experience from trial managers. Trial managers were asked to identify reasons why sites might fail to recruit to target in randomised and non-randomised interventional trials.


ESP identified eight possible 'red flags' to predict recruitment failure at a site:
• previous poor site performance
• slow approvals process
• strong staff/patient preferences
• the site recruitment target
• the trial protocol and its implementation at the site
• lack of staff engagement
• lack of research experience among site staff; busy site staff.

We have used these identified flags to develop a site performance prediction tool. We hope that using this tool will improve the accuracy of trial managers’ predictions about site recruitment performance, but this needs evaluation. It is this evaluation that ESP2 will conduct.

To evaluate the tool, we need trial managers to make 1000 site performance predictions (will the site recruit to target yes or no?). We invite trial managers working in anywhere in the world to register and start making predictions! If you’d like more information, scroll down.

Can I take part and registration

You can take part in ESP2 if:

  1. You are responsible (e.g. as a Trial Manager, Trial Manager Assistant or Clinical Research Associate) for setting up trial recruitment sites and carrying out site initiation training with site staff.
  2. Your trial is currently opening new recruitment sites. You will be able to make predictions between October 2019 and until September 2023.
  3. Your trial is testing one or more interventions. Your trial can be randomised or non-randomised but cannot be cluster randomised.
  4. You and your trial recruitment sites can be anywhere in the world.
  5. You are willing to make at least one prediction. There is no limit to the number of predictions you can make

ESP2 started in 2019 and is expected to run until 2024.

What is this study about?

Taking part in ESP2 would involve:

  1. Registering yourself and your trial(s) in a one-off simple registration on this website.
  2. Making a recruitment prediction on this website as soon as possible after completing site initiation training. The site does not need to be open for recruitment when the prediction is made, so long as the site initiation training has been done.

  3. You can make just one prediction, or many more, it’s up to you. We just ask that you enter all information needed for each prediction. Entering your prediction will take less than 10 minutes.
  4. The ESP2 team will be in contact 6 months later and then again 12 months later to see how recruitment actually went at each site for which you made a prediction..
